CSBG Case Manager

General Summary

Responsible for delivering direct service support and referrals through the Community Services Block Grant (CSBG) and related programs.  Provides CSBG case management services for designated families, to include the planning, documentation and follow-up of services in accordance with the Integrated Assessment and Case Management Service Plan.  Coordinates access to necessary services, materials, programs and information for all client families. 

Primary Responsibilities

1. Performs in-take and assures that all documentation is provided for applications for assistance for the CSBG and other related programs.

2. Responsible for the completion of the Pre-Assessment and Integrated Assessment forms for CSBG participants; responsible for the implementation and management of the Case Management Service Plan for each assigned family.

3. Monitors participants progress through direct client contact, phone calls, email and/or other preferred methods. 

4. Responsible for documenting family progress by preparing, updating and maintaining progress notes and goals on the Activities and Contact Record on a weekly basis. 

5. Provides information and referrals; assists families in gaining access to needed medical, social, educational, career, developmental and other appropriate services, including transportation.

6. Responsible for the entry of client data and the use of the data for case management services and reporting. Must be entered into the designated software utilized by the CSBG program. 

7. Responsible for meeting CSBG program targets and goals.

8. Assists in the preparation of programmatic reports monthly. 

9. Responsible for establishing collaborative relationships with community partners that may assist clients in meeting their family needs and goals. 

10. Other duties as assigned by the Director of Community Services and Development

Work Experience

  • 3 years minimum employment experience in social services or case management
Education/Certifications/Licensure

  • Bachelor’s Degree Preferred or Associates degree in related field
  • Emphasis in social work or case management preferred
Skills - Required

  • The candidate must have effective communication and people skills and excellent computer skills.
  • Must have strong oral and written communication skills, along with effective interpersonal abilities. 
  • Must demonstrate professionalism and the ability to work efficiently in a fast-paced environment. 
  • Must have intermediate computer skills.
  • Must be able to perform multiple tasks simultaneously
  • Must be able to meet multiple deadlines consistently.
  • Must have sound judgement and decision-making abilities and able to work closely with others.
Skills - Preferred

  • Healthy knowledge of referral sources in surrounding areas. 
  • Bilingual in English/Spanish preferred 
Physical Requirements

Medium to heavy physical duties include standing, walking, reaching, bending, hearing, talking for up to (8) hours a day. Must be able to lift forty (40) pounds. Must be able to pass a pre-employment physical examination.
